Earth Care Preferred Provider Program Application

Instructions:

  1. Complete this application.

  2. Sign it.

  3. Return it with a copy of your government-issued ID. Acceptable IDs: Driver's License, State ID, USA or Foreign Passport, Student ID, Military ID.

  4. A paper application can be requested by emailing sales@earthcarebrands.com.

Processing Time: Your application will be processed within 24 hours of receipt.

Independent Contractor Agreement: By signing this agreement, you agree to join the Earth Care Preferred Provider Program as an independent contractor, subject to the terms of the Preferred Provider Agreement.

Applicant Certification: I certify that my answers are true and honest to the best of my knowledge. I understand that any false or misleading information in my application may result in termination of employment.

Applicant Rights: As a contractor, you are entitled to the following rights, provided you comply with the provider agreement:

  • Same-day gas reimbursement: 10 cents per mile locally, 25 cents per mile long-distance.

  • Flexible scheduling.

  • Exclusive assignment placement.

  • Same-day reimbursement for all materials upon successful job completion.

  • Free phone support/assistance.

  • Cancellation fee if a job is canceled or not as described upon arrival.

  • No unpaid job viewing; all calls are paid jobs.

Conditions for Removal from Provider Network: The following actions may lead to removal from the provider network and possible further action:

  • Not showing up for an assigned job.

  • Walking off a job.

  • Receiving payments outside the platform.

  • Refusing to check in or hanging up on an Earth Care representative.

  • Refusing to provide required before and after pictures.

Applicant Responsibilities:

  • Responsible for own transportation, tools, and insurance.

  • If lacking liability insurance, Earth Care’s insurance is available at $19 per month or 10% of each completed job.

  • Responsible for anyone brought to the job site; Earth Care is not liable for any injury they sustain.

  • Payment will be in cash on the same day after job completion.

Work Order Process: Work orders will be routed via our app or SMS with:

  • Date/Time

  • Pay Amount

  • Location

  • Job Description

To accept a job, reply to the text indicating your acceptance. Jobs are assigned on a first-come, first-served basis. For support, contact our pro support line at 1-833-736-8835.

Upon Arrival:

  • Call Earth Care pro support to complete the check-in process.

  • Take before and after pictures for documentation.

Payment and Reimbursement:

  • Ensure the client makes the payment upon job completion.

  • For reimbursement of parts used, present a copy of the receipt.

Important Guidelines for Success:

  • If unsure about a task, contact us for assistance while maintaining professionalism in front of the client.

  • Refer to yourself as an Earth Care representative, not a subcontractor.

  • Purchase A Uniform

  • Do not advertise other service providers without written permission from Earth Care.
